HCFA offers on-line enrollment forms

     The Health Care Financing Administration office is offering on-line enrollment forms for Medicare.  Medicare forms 855, 855A, 855B, 855R and 855S can be downloaded from their site.  Other information is also available,  including a detailed user's guide.  Forms cannot be submitted electronically at this time, but printed out and mailed.  Visit the HCFA website for forms and additional information:  www.hcfa.gov/medicare/enrollment/forms
