The Academy of Medicine of Cleveland & Northern Ohio Physician Charity Care Survey

The AMCNO has developed a survey to gain a better understanding of the level of uncompensated care (i.e., charity care) by physicians to patients in this region.

In addition, the AMCNO wants to know more about physicians’ perceptions of activities related to quality of care and public reporting. Please take a few moments to complete and return this survey. All responses will be anonymous and will be reported in aggregate so that no given individual is identifiable.
