The deadline to complete and return the proof of claim form for the Aetna proposed settlement is Sept. 30. If you do not submit a proof of claim form to the settlement administrator prior to Sept. 30, the portion of the settlement fund that you are entitled to receive will be donated to a charitable foundation that has been established in connection with the settlement of the class action.
