Aligning Forces for Quality Projects Expands

Last week the Cleveland Plain Dealer outlined information on the national launch which is designed to expand the Robert Woods Johnson Foundation Aligning Forces 4 Quality project. In the near future, the Cleveland group - of which the Academy of Medicine of Cleveland & Northern Ohio is a partner will release a report showing how physician practices and their patients managed diabetes in 2007. Three hundred physicians, including those affiliated with Cleveland's largest hospitals, and 24,000 diabetic patients are involved. The expansion will further fund work on racial and geographic variations and transitions in care adding nurses and hospitals to the team.
