AMCNO Continues Involvement in the Better Health Greater Cleveland project

In 2007, the Robert Wood Johnson Foundation created regional partnerships to improve the health of people with chronic conditions. Better Health Greater Cleveland (BHGC) is one of the 14 communities supported by the program, called Aligning Forces for Quality.

BHGC has just released its’ second Community Health Checkup report. They report the achievement of 42 primary care group practices with more than 25,000 adult patients with diabetes. Included among these are 33 practices that reported their results in the first report, providing an opportunity to view some changes in region-wide achievement.

However, the report continues to show disparities in outcome achievement among those with fewer resources in our community. Poorer outcomes were seen among non-white and poorer patients, as well as uninsured patients, whose achievement was both lower than insured patients and showed no improvement over the two measurement periods.

Dr. Cebul, the medical director of the project, recently attended a meeting of the AMCNO board of directors to provide additional background on this report. Dr. Cebul is also preparing an article for an upcoming issue of the Northern Ohio Physician magazine outlined the findings of the second report.
