University Hospitals Faculty Services (UHFS) and the Academy of Medicine of Cleveland/Northern Ohio Medical Association
(AMC/NOMA) invite you to attend a workshop addressing Evaluation/Management (E/M) documentation guidelines, medical necessity, use of modifier 25, Teaching Physician documentation requirements and much more!
You may select to attend any/all of the (4) sessions over the span of two days and can receive CMEs or
CEUs, as appropriate. This event is presented by PalmettoGBA (Ohio’s Medicare Carrier) staff.

Session A: E/M For Physicians and Non-Physician Practitioners – Provides in-depth understanding of E/M documentation requirements, review examples, and error data for various specialties!

Session B: Lunch and More: E/M Practicum – A highly interactive session for review of documentation samples in selecting various levels and types of E/M services!

Session C: M.M.I.T. – In depth discussion of medical necessity, use of 25 modifier, “Incident-to”, teaching physician documentation requirements and much more!
